Process

.DIN surrender application (Form DIR-5)
• Proof of identity (PAN, Aadhaar, Passport, etc.)
• Proof of residence
• Copy of DIN allotment letter (if available)
• Affidavit/Declaration for surrender
• In case of duplicate DIN – Both DIN details must be provided
• In case of death/incompetency – Death certificate / Court order
• Board resolution of resignation (if applicable)

1. Prepare DIR-5 Form – The official MCA form for surrender of DIN.
2. Attach Documents – ID proof, address proof, declaration/affidavit, and supporting documents.
3. Digital Signature – Form must be signed with the applicant’s DSC.
4. Certification – Attested by a practicing CA/CS/CMA.
5. Submit on MCA Portal – File DIR-5 online with prescribed fee.
6. MCA Review – ROC verifies the application and supporting documents.
7. Approval – DIN is deactivated/cancelled, and a confirmation is issued.
